Who
is eligible?
We are particularly concerned to avoid people continuing medication
indefinitely without good reason. Therefore, we only place
patients on a programme for obtaining repeat prescriptions
after discussion with the doctor. At that discussion we determine
which drugs will be provided and the frequency of repeat prescriptions.
In the vast majority of cases it is necessary for the patient
to attend for review if six months have elapsed since the
previous medication.
Please
order your medication in good time.
Requests
for repeat prescriptions must be made in writing to avoid
mistakes.
Procedure
Our procedure for obtaining repeat prescriptions is computerised.
It takes two working days. We do not take telephone requests
for repeat prescriptions.
To obtain a repeat prescription, you can either:
:: Fax your request to us on 476 0931.
:: Post a written request to us with a stamped
.. addressed envelope.
:: Place a written request in the special
box provided

If you wish for a chemist to collect the prescription for
you, you must contact the chemist yourself to arrange this.
There is a dispensing chemist situated within the health
centre.

You can request a repeat prescription by filling in the
form below.
The information collected by this form is used to help us
validate your request and will not be used for any other
purpose. You must supply your unique Confidential Reference
Number (Patient Number), which can be found on the top of
your prescription counterfoil.
