the best possible care available.
Your
Rights Are:

To be registered with a family medical practice.
:: To have all the medicines
on prescription which your

doctor feels are necessary and appropriate.
:: To be referred to
a consultant when your family doctor

thinks it is necessary and to be referred for a second

opinion if you and your family doctor agree this is

desirable.
:: To have access to
your own health records, subject to

any limitations in law.
:: To know that those
working for the NHS are under a
::
legal duty to keep the contents of your health records
::
confidential.
:: To choose whether
or not to take part in medical
::
research or medical student training.
:: To receive a copy
of the doctor's practice leaflet,
::
setting out the services he or she provides.
:: To receive a full
and prompt reply to any complaint you
::
make about NHS services.
:: To change your doctor
quickly and easily (where the
::
doctor/patient relationship has broken down irretrievably
::
the doctor also has the right to remove you from his/her
::
list).
Your
Responsibilities:
We
can help you best if you:

Treat staff politely and with courtesy as you would wish

to be treated yourself.
:: Ask about anything
you don't understand.
:: Realise that the
demand for medical services is

unpredictable so at times the doctor is very busy and
::
may not be running to time.
:: Tell the surgery
straight away if you change your name,
::
address or telephone number - it helps us keep our
::
records up to date.
:: Understand that the
results of tests are only given to
::
the person who had the tests, or in the case of
::
children, the parent or guardian.
:: Attend on time for
your appointments and give adequate
::
notice if you cannot keep an appointment.
:: Please ensure children
are well behaved, as clearly some
::
of our patients are very ill.
